Nuestra huella médica se ha vuelto tangible gracias a invitaciones constantes a hospitales y centros de todo el país, como el Hospital Kokilaben y el Instituto de Investigación Médica, donde nuestro querido Director, el Dr. Ashish Katewa, recientemente presentó un seminario o visitas de eminentes médicos de K. 'Chirayu Chhattisgarh' Hoy, el 17 de julio de 2018, tuvo lugar la gran inauguración del Centro de Excelencia Chirayu Chhattisgarh en Cuidado y Entrenamiento Cardíaco Pediátrico por parte del Honorable Ministro Principal de Chhattisgarh, Dr. Raman Singh. 'Chirayu Chhattisgarh' es una iniciativa conjunta del Gobierno de Chhattisgarh y el Hospital Sri Sathya Sai Sanjeevani, Naya Raipur, y es el primer centro colaborativo de la India para la Excelencia cardíaca pediátrica. La palabra 'Chirayu' en hindi significa 'larga vida'. Para innumerables niños en todo el estado, nacidos con corazones frágiles, 'Chirayu' sigue siendo un sueño lejano. Es para convertir el sueño en una realidad que este Centro de Excelencia ha sido establecido. Además de ampliar su trabajo de realizar más cirugías cardíacas para niños sin costo, el Equipo Sanjeevani también tiene como objetivo proporcionar la capacitación necesaria a una amplia gama de trabajadores de salud de primera línea del estado. PACIENTES PEDIÁTRICOS en julio de 2018 Este mes se realizaron un total de 1405  ECHOs y 65 Procedimientos intervencionales decateterizaciónen Naya Raipur y  774 ECHOs y 27 Procedimientos intervencionales decateterizaciónen Palwal. ECHO hechos en Palwal en julio de 2018 ECHO hechos en Raipur en julio de 2018 CIRUGÍAS CARDIACAS PEDIÁTRICAS (julio de 2018) Se realizaron 100  cirugías cardíacas pediátricas en julio de 2018 y se realizaron 4731  cirugías en el Hospital Sri Sathya Sai Sanjeevani, Naya Raipur desde el inicio en noviembre de 2012, con pacientes procedentes de todo el país y otros países en desarrollo. Se realizaron 37  cirugías cardíacas pediátricas en julio de 2018 y se realizaron 448  cirugías en el Hospital Sri Sathya Sai Sanjeevani, Palwal, Haryana, desde abril de 2017. Cuarta Actualización de Anestesia Cardíaca Pediátrica, Cuidados Intensivos y Perfusión - 2018 Presentado en el Hospital Kokilaben Dhirubhai Ambani, Mumbai El Dr. Ashish Katewa , Director y Jefe de Cirugía Cardio-Torácica, el Hospital Sri Sathya Sai Sanjeevani y el Centro de Atención Infantil del Corazón, Naya Raipur intervinieron en el Hospital e Instituto de Investigación Médica Kokilaben Dhirubhai Ambani con motivo de la  Cuarta Anestesia Cardíaca Pediátrica, Cuidados Intensivos y Perfusion Update-2018 celebrada el 30 de junio y el 1 de julio de 2018 en Mumbai. El área de enfoque clave fue la comunicación entre el cirujano y el perfusionista en los momentos actuales. La conferencia semestral tenía como objetivo atender a perfusionistas e intensivistas que hacen hincapié en la importancia de la comunicación y las medidas para evitar la negligencia médica. K Women & Children's Hospital Singapur visitaron Naya Raipur como parte del Programa de Educación Médica Continua para el personal de enfermería y otros paramédicos del hospital los días 10 y 11 de julio de 2018 . El equipo discutió varios aspectos de la salud y la capacitación maternoinfantil; cuidado perioperatorio seguro para pacientes cardíacos; una introducción al entorno y la cultura de la Unidad de Cuidados Intensivos de K. This month, the Sri Sathya Sai Sanjeevani Centre for Child Heart Care, Naya Raipur, Chhattisgarh has partnered with the Government of Chhattisgarh in an initiative called 'Chirayu Chhattisgarh" which recognizes the hospital as a Center of Excellence in Paediatric Cardiac Care & Training. The Chief Minister of Chhattisgarh, Dr. Raman Singh inaugurated the center early this month. It is truly remarkable how the different limbs of Sai Sanjeevani work together in moving the mission forward, enabling the treatment of 55,839 Pediatric Cardiac Consultations,  6,091 pediatric cardiac surgeries and cath interventions and 13,112 rural children & 145 rural pregnant mothers screened - All Totally Free of Cost. Work tirelessly continues for the Divine Mother & Child Health Team who are ceaselessly following mission protocols to screen and educate every last child in rural and urban India. Our Medical footprint has now become tangible with constant Seminar invites to Hospitals and Centers across the country, like at Kokilaben Hospital & Medical Research Institute where our dear Director, Dr. Ashish Katewa recently presented a seminar, or visitations from eminent Physicians from K. K Women's & Children's Hospital, Singaopre, conducting a Continued Medical Education Program for our Nursing staff. Our Newsletter this month also caries two medical snippets along with a warm adieu to two members of the Sai Sanjeevani family. 'Chirayu Chhattisgarh' Today, the 17th of July 2018, saw the grand inauguration of ‘Chirayu Chhattisgarh Centre of Excellence in Pediatric Cardiac Care & Training’ by Honorable Chief Minister of Chhattisgarh, Dr Raman Singh. 'Chirayu Chhattisgarh' is a joint initiative by the Government of Chhattisgarh and Sri Sathya Sai Sanjeevani Hospital, Naya Raipur and is India’s first collaborative centre for Pediatric Cardiac Excellence. The word 'Chirayu' in Hindi means 'long life'. For countless children across the state, born with fragile hearts, 'Chirayu' remains a distant dream. It is to convert the dream into a reality that this Centre of Excellence has been established. Apart from its expanding work of performing more child heart surgeries free of cost, Team Sanjeevani also aims to provide necessary training to a wide range of frontline health workers of the state. INAUGURATION The day's program commenced with Dr. Raman Singh (Chief Minister of Chhattisgarh), Shri T B Jayachandra (Former Minister for Law, Parliamentry Affairs & Irrigation, Karnataka), Shri C. Sreenivas (Chairman, Sri Sathya Sai Health & Education Trust) and numerous delegates from various parts of the country and world, walking through the newly established Surgical & ICU Complexes that were set-up in a record 3-month period. Cheif Minister's Address The dignitaries then convened at the C. Rajeshwari Auditorium in Sai Sowbhagyam where a visibly moved Dr. Raman Singh had these words to share, “I congratulate the entire team of the hospital for this new beginning. Can the State ever be free of Congenital Heart Disease? I am certain, with partners like Sri Sathya Sai Sanjeevani, this is absolutely possible! In fact, just give me the number of Operation Theatres required and I will take care of everything else. This is truly a divine and sacred space. Over 5000 surgeries have been conducted on children from all over the world! Giving the Gift of Life to a child is the most sacred of acts. On behalf of entire Chhattisgarh, I heartily congratulate the team. I was also presented with the ‘ViScope-Stethoscope’, which is sure to be an important tool to help eradicate Congenital Heart Disease in the country. I have seen the entire facility and everything here is world-class! When anyone visits the State, I always make sure I tell them, it does not matter which other places you visit, but you MUST see Sri Sathya Sai Sanjeevani Hospital before you leave. It is a direct manifestation of Baba’s Will and Divine Blessing.” Truly, this day marks yet another important milestone in the sacred mission of Sai Sanjeevani - totally free of cost, quality, and compassionate child heart care.  We present to you a consolidated report of all the work done at Sri Sathya Sai Sanjeevani Centers for Child Heart Care at Naya Raipur, Chhattisgarh & Palwal, Haryana.            To offer a little perspective, the numbers you see are not mere figures representing the volume of work being done. What you see is workings of a magnificent system running on an ethereal force beginning with a group of individuals whose choice was to commit to the mission of lightening the burden of Congenital Heart Disease, to the hospitals that provide this care and finally to the child that has been healed. To us these are not just numbers, they are promises of a healthier world. PEDIATRIC OUTPATIENTS in July 2018 A total of 1405 ECHOs & 65 Interventional Cath Procedures in Naya Raipur and 774 ECHOs & 27 Interventional Cath Procedures in Palwal, were performed this month, A myriad of cases are tackled. Also the following chart highlights the work of the specialty clinics done so far at the Naya Raipur Center. ECHOs done in Palwal in July 2018          ECHOs done in Raipur in July 2018 PEDIATRIC CARDIAC SURGERIES  (July 2018) 100 Pediatric Cardiac surgeries were conducted in July 2018 and 4731 surgeries have been conducted at Sri Sathya Sai Sanjeevani Hospital, Naya Raipur since Inception in Nov 2012, with patients coming from all over the country and other developing countries. 37 Pediatric Cardiac surgeries were conducted in July 2018 and 448 surgeries have been conducted at Sri Sathya Sai Sanjeevani Hospital, Palwal, Haryana since April 2017. Divine Mother & Child Health Program           The Divine Mother and Child Health Program, under the Department of Public Health and Research has recently completed 5 years since inception. It has already touched thousands of lives impacting pregnant women and children in rural areas across 4 states (Chhattisgarh, Haryana, Karnataka & Andhra Pradesh) in India.           The Mother Support Group, started by the Department of Public Health and Research under the Divine Mother and Child Health Program was conducted in the Village Parsada, Arang Block in Chhattisgarh. This was attended by the expecting women, the new mothers and the family members. An interactive game of snakes and ladders was conducted to explain to them about the dos and don’ts during pregnancy. The importance of breastfeeding and the correct ways of the same were explained to the women and their family members to ensure a healthier start in life. 4th Paediatric Cardiac Anaesthesia, Intensive Care & Perfusion Update - 2018 Presented at Kokilaben Dhirubhai Ambani Hospital, Mumbai Dr Ashish Katewa, Director and Head of Cardio-Thoracic Surgery, Sri Sathya Sai Sanjeevani Hospital and Centre for Child Heart Care, Naya Raipur spoke at Kokilaben Dhirubhai Ambani Hospital & Medical Research Institute on the occasion of the 4th Pediatric Cardiac Anaesthesia, Intensive Care and Perfusion Update-2018 held on 30th June & 1st July 2018 in Mumbai. The key area of focus was the communication between surgeon and perfusionist at current times. The biannual conference was intended to cater to perfusionists and intensivists stressing on the importance of  communication and measures to avoid medical malpractice. Dr Ashish Katewa, in his presentation outlined the unique characteristics of the environment in which the surgeon and the perfusionists work. The primise being that both are engrossed in their work; both cannot see what the other is doing and most often each constantly needs information from the other. He laid emphasis on the fact that effective communication is a precondition of effective teamwork. He added that the breakdown in communication has been linked to worse outcomes in the operating room. He presented a few specific research points to elaborate on the same. Retrospective review of 16000 hospital deaths found that communication errors were twice as frequent as errors due to inadequate clinical skills- Wilson RM et al.           The quality in Australian healthcare study. Med. J Aus                     1995; 163:458-71 In a review of 444 surgical malpractice 81 communication breakdowns were identified 92% of these were verbal interchange or events- Greenberg CC et al. Patient communication breakdowns resulting in injury to surgical patients. J Am Coll Surg. 2007; 204:533-40 Incomplete, nonexistent or erroneous communication was casual factor in 43% of surgical adverse events. – Gawande AA et al. Analysis of errors reported by surgeons at three teaching hospitals. Surgery. 2003; 133:614-21 Nursing CME in Maternal & Child Health A team of doctors, nurses and administrators from K. K Women & Children’s Hospital Singapore visited Naya Raipur as a part of Continuing Medical Education Program for the nursing and other paramedical staff of the hospital on the 10th and 11th of July, 2018. The team discussed, various aspects of maternal and child health and training; safe peri-operative care for cardiac patients; an introduction to K. K Women & Children’s Intensive Care Unit setting and culture; quality and outcome measures; they also discussed the workings of setting-up a training program and what entails post-op cardiac nursing care. The CME ended with a case presentation. The team was led by Professor Annette Jacobson, Director, International Medical Program, Associate Dean and Clinical Educator Lead Medicine K. Let's tell you a story... Everyday countless children and parents pass through the gates of Our hospitals, some armed with confidence that treatment here is definitive, some in distress of having to go through Hospital protocols yet again, many as a last resort because they've faced too many closed doors. But on leaving, there is only one emotion. The deepest love that challenges the very essence of the human spirit. We see everyday, what that love translates to. Here are individuals with crushing financial constraints, geographical restrictions, language barriers, cultural differences who experience a change so deep that their debility converts to a resilient force to do better. It is humbling to see the capacity of a person driven by Love. Here are some cases that have piqued medical interest.           Situs, Inversus, Dextrocardia - Device Closure of PDA           Date : 20/07/2018 Isolated dextrocardia with situs inversus is an extremely rare type of situs anomaly with an estimated incidence of 1 per 22,000 in the general population.  A 3 year old girl Harshita had an accidental diagnosis of isolated dextrocardia with situs inversus while being evaluated for continuous heart murmur. Echo examination of the patient identified the presence of patent ductus arteriosus from right aortic arch to RPA. Successful transcatheter closure of patent ductus arteriosus was performed with 8/6 Lifetech PDA occluder. In keeping with the cath lab strategy of ALARA, the procedure was performed within 15 minutes from obtaining venous access and a fluro time of 2 minutes only.  Interestingly this case was also done with only femoral venous access without arterial sheath.  This case report is worth reporting both, for demonstrating the possibility of the percutaneous device closure of the patent ductus arteriosus in patient with this unusual situs anomaly to interventional cardiologists and because of scanty of literature. THE STORY OF MAMATA Mamata, 11-year old girl from Palwal, Haryana She was brought to our hospital with complaints of breathing difficulties over last 1 year, with exertional palpitations. She was diagnosed to have a Large Fossa Ovalis (16mm x 14mm) ASD and upper muscular (5mm) VSD. In view of suitability for Percutaneous closures, she was admitted for the same. Initially, Mamata underwent Device Closure of VSD initially with (8/6mm) ADO. After this, she underwent Device closure of ASD with ASO (20mm) in th same sitting. Both procedures were done in TEE guidance under General Anaesthesia. It was an extremely satisfying procedure to have done both VSD & ASD closure in the same sitting over a span of one hour. Mamata recovered very well, she was observed over the next 36 hours and discharged soon after. The family paid obeisance to Swami for offering them a new lease in life and an opportunity to empowering their little girl.   Sai Sanjeevani in print This month we would like to show case writings on and by Sri Sathya Sai Sanjeevani. "Having travelled all the way from the exotic continent of Africa to Incredibly India on a quest for knowledge & skills to earn a Fellowship in Pediatric Cardiology; I must say the experience in SSSSH so far has been most heartwarming and quite illuminating!                     The patient load is very heavy, so my skills acquisition and cognition on use of echocardiography for diagnosis and follow-up of children with congenital heart diseases has taking off smoothly, albeit with some turbulence at handling that "Echo probe" and seeing the human heart in shades of gray!!             The Medical Team, Administrative and Human Resource staffs are simply awesome in providing me with guidance through my baby-steps in "CHILD HEART," and Chhattisgarh India! Adal 'Baba' Sagar, as he is fondly called, always had the largest heart and brightest smile on campus. He joined the Sai Sanjeevani family on the 1st of March 2013 at Naya Raipur as a housekeeping staff. He was a man on a mission, always at work, whether it was in the constant upkeep of our beautiful campus or lending kind, quiet words of comfort to a distressed family or fervently scrubbing floors every Sunday morning. No amount of work was too much or too time consuming for Adal Baba, even on days off he would be present for any social program being conducted and when all was done, without being asked, cleaned up after everyone left. Adal baba has now reached his retirement age. He is the first staff member of Sai Sanjeevani to retire from service. We will dearly miss seeing him hanging from the tallest ladder and his warm greetings as we pass him by in the corridors, specially on a hard day. Adal baba was given a heartfelt send-off by Swami and our beloved Chairman Mr C Sreenivas on their recent visit to Naya Raipur. We wish Adal baba a very happy retired life! . We wish you all the Love & Luck Maryanne! Dr Maryanne Koraai- Pediatric Cardiology Trainee from Suva, Fiji Dr Maryanne Koraai is the first trainee in Pediatric Cardiology from Suva, Fiji. She works as a Pediatric Registrar with the Ministry of Health, Fiji. The medical team from Sai Sanjeevani has visited Fiji Islands twice to perform surgeries for the children from South Pacific. It has been observed that, in addition to medical and surgical outreach programs in Fiji for children, institution building and training of local doctors was necessary to help in quicker diagnostics in Pediatric Cardiology and make progress in addressing the issue of Congenital Heart Disease. Dr Maryanne Koraai was here for a period of 3 months in Naya Raipur, during which she worked in the Cardiology department and had her experience and training in the Out-Patient Department. She became an integral part of the Sai Sanjeevani family and had to bid farewell to her second home.
