Cada año nacen en Fiji 200 niños con defectos cardíacos congénitos .  La mayoría de estos no se diagnostican y, por lo tanto, no se tratan. Fiji depende de los equipos visitantes de Australia, Nueva Zelanda e India para el cuidado cardíaco de sus hijos. Un pequeño número de niños también son enviados al extranjero para operaciones más complejas. Un estudio observacional no publicado realizado en el Hospital Colonial War Memorial (CWM) durante el período de cinco años a partir de 2012 y 2016 ofrece una perspectiva del problema. Durante este período, 200 niños ingresaron con CHD tratable en el hospital CWM. Solo 46 niños fueron operados, 96 murieron y los 68 restantes están en espera de tratamiento. vida. Gracias" UN CASO MÉDICO RARO Retorno venoso pulmonar anómalo total (TAPVR) y ventana aortopulmonar (APW)           Little Gudiya se presentó en Sri Sathya Sai Sanjeevani, Naya Raipur con quejas de dificultad para respirar y mala alimentación. Pesó solo 3 kg a los 4 meses de edad. El ecocardiograma reveló que tenía un gran agujero entre las dos grandes arterias que surgen del corazón. Este aproximadamente el 0.2% de todas las lesiones cardíacas congénitas y un centro activo puede esperar aproximadamente un caso por año. El retorno venoso pulmonar anómalo total es otro defecto poco frecuente con una incidencia del 1,5% de todas las lesiones cardíacas congénitas. This April marked the beginning of an exciting new chapter in the global mission of Sai Sanjeevani, with the inauguration of the Sri Sathya Sai Sanjeevani Medical Centre in Fiji, in collaboration with the Sai Prema Foundation Fiji. This medical centre was officiated by Honorable Prime Minister Voreqe Bainimarama, Government of Fiji, who was overjoyed by the work of Sai Sanjeevani and Sai Prema Foundation. The centre will provide free of cost outpatient services, and the vision is for the centre to become the first international facility for Sri Sathya Sai Sanjeevani’s child heart care services by November 2019. In the week leading up to this inaugural function, a team of doctors, nurses, and other medical staff from Sai Sanjeevani Raipur had visited Fiji to perform 26 free of cost surgeries. The Prime Minister also mingled with the children whose little hearts were successfully operated upon. With these new initiatives, Sai Sanjeevani’s reach will broaden further. It has already embraced 25885 outpatients and completed 5339 surgeries and interventions! Sai Sanjeevani continues to bring renewed hope for many, as evidenced by the stories of Constable Gabbar Singh, a single father who sought treatment for his son at Palwal after the demise of his wife, Divyanshi Verma, who was saved in the nick of time after collapsing upon arrival at the Palwal hospital, and Little Gudiya who was treated for a rare medical condition not previously seen in medical literature. The technology at Sai Sanjeevani continues to improve in an effort to provide top-quality healthcare completely free of cost. In Naya Raipur, Arrhythmia Management Services commenced this month to further this cause. The impact of Sai Sanjeevani is also being acknowledged by national publications, like IndiaMedToday which published a big feature on the sustainable and high-quality model of Sanjeevani hospitals, contributed by Dr. Ashish Katewa, Director, Sri Sathya Sai Sanjeevani Hospital, Naya Raipur. The Divine Mother and Child Health Programme continues to bring information, awareness, and medical help to those in need. This includes screening for pregnant women, daily parental outreach sessions, and even a blood donation drive. Your Sai Sanjeevani Team SURGERIES (April 2018)   105 Pediatric Cardiac surgeries - 84 at Sai Sanjeevani, Naya Raipur and 21 as part of the Overseas Mission in Fiji were conducted in April 2018 and 4421 surgeries have been conducted at Sri Sathya Sai Sanjeevani Hospital, Naya Raipur since Inception in Nov 2012, with patients coming from all over the country and other developing countries. OUT PATIENT SERVICES (April  2018) 1117 outpatient registrations were recorded during the month - 628 new pediatric cases, and 489 revisit and followup pediatric cases. 758 Patient Pre registrations were recorded this month. Out of a total of 1464 ECHOs performed this month, 628 are new pediatric cardiac cases as shown in the table below. Also347 post-operative inpatient & TEE screenings and 489 revisit and medical follow up ECHOs were performed.   INTERVENTIONS (April  2018) 46 Pediatric Cardiac Interventions were conducted in April 2018.   PEDIATRIC CARDIAC OUTPATIENT SERVICES  (Nov 2016 - Apr 2018) Since the inception of the Hospital in November 2016, 4052 pediatric cardiac outpatientshaven been screened and managed with patients coming from 22 states of the country - Assam, Haryana, Delhi, Uttar Pradesh, Bihar, Gujarat, Jharkhand, Jammu & Kashmir, Odisha, Punjab, Himachal Pradesh,  Manipur, Rajasthan, Uttarahand, West Bengal, Chhattisgarh, Andhra Pradesh, Maharashtra and Madhya Pradesh. 352 New Pediatric Outpatients were managed in April 2018.                                                            PEDIATRIC CARDIAC SURGERIES AND INTERVENTIONS  (Apr 2017 - Apr  2018) The first Pediatric Cardiac Surgery at the Centre was conducted on 18 April 2017.332 Pediatric Cardiac surgeries have been conducted since April 2017 at Sri Sathya Sai Sanjeevani International Centre for Child Heart Care & Research, Baghola, Palwal, Haryana.409 Pediatric Cath Interventions were conducted till Apr 2018. ---------------------------------------------------------------------------- Pediatric Cardiac Surgeries (Apr 2018) Pediatric Cardiac Interventions (Apr 2018)                                                 All Services are rendered Totally Free of Charge               Fiji is an archipelago of 330 island spread across the south pacific ocean. Its population is 900,000, 87% of which lives in two major island of Viti Levu and Vanua Levu. Despite being the most developed economy in South Pacific, Fiji lacks quality healthcare. Every year 200 children with congenital heart defects are born in Fiji. Most of these remain undiagnosed and thereby untreated. Fiji relies on visiting teams from Australia, New Zealand and India for cardiac care of her children. A small number of  children are also sent overseas for more complex operations. An unpublished observational study performed at Colonial War Memorial (CWM) Hospital over the period of five years from 2012 and 2016 gives a perspective of the problem. During this period, 200 children were admitted with treatable CHD at CWM hospital. Only 46 children were operated, 96 died and the remaining 68 are awaiting treatment. First Overseas Child Heart Care Initiative Sri Sathya Sai Sanjeevani Hospital was invited in February 2016 by the Fiji Ministry of Health and Medical Services to understand the problem and provide a longterm sustainable solution. A Paediatric Cardiac team visited Suva, Capital of Fiji in August 2016 in collaboration with Sai Prema Foundation Fiji. During this visit, 73 children were evaluated, 25 of these were operated in Fiji and 4 children were subsequently operated in India. An offer was also made to the ministry to enable the training of local doctors, nurses and other members of a cardiac team at Sri Sathya Sai Sanjeevani, Naya Raipur. 2nd Child Heart Care Mission to Fiji A second humanitarian visit to Fiji was made in April this year. This was truly an international team comprising of members from India (7), Australia (4), USA (2) and Oman (1) led by Dr Ashish Katewa, Director, Sri Sathya Sai Sanjeevani Hospital, Naya Raipur. The team evaluated 84 children with CHD. 26 operations were performed, including complex repairs for TAPVC, Pulmonary Atresia and Ebsteins Anomaly. These procedures were performed for the first time in Fiji. Another history of sorts was created, when the first paediatric cardiac interventions were performed in whole of south Pacific. In addition, regular lectures were delivered for the local trainees. It has been long recognised that, visiting humanitarian teams, though providing much needed care, are not the answer to the problem. Fiji needs its own paediatric cardiac service which is available to the children of Fiji for 365 days a year. An important step has been taken in that direction.  Sai Prema Foundation (SPF), a Fiji based NGO has announced that a pediatric cardiac hospital will be established in Suva by the end of 2019, under the guidance of Sr Sathya Sai Sanjeevani Hospital, India and support from Ministry of Health and Medical services, Government of Fiji.  This will be the first pediatric cardiac hospital in South Pacific and will serve not only the children of Fiji, but also all other island nations in South Pacific. Dr Mahendra sees himself actively contributing in the field of genetic sciences with passion for research in congenital cardiac sciences for the betterment of humanity.  After clearing my basic schooling with meritorious scholarship, I started my journey as a medical professional at Indira Gandhi Government medical college, Nagpur. There I groomed up to become a young doctor and bagged prestigious scholarships which took me to Germany and France to represent our nation as an international fellow to get trained in basic Paediatric Science. Upon returning, I completed my training in paediatrics from Government Medical College, Latur. To further continue my journey, I moved to the national capital to get trained in congenital cardiac sciences at Indraprastha Apollo Hospital, New Delhi under renowned and well-known guides and mentors of our country.          After finishing my advanced training in congenital cardiac sciences, I joined the mission of caring for the little hearts of the country and moved to Sri Sathya Sai Sanjeevani International Centre for Child Heart Care and Research at Palwal to be part of a world class team.   I really feel blessed and thankful to the almighty for introducing me to this temple which is taking care of tender hearts and providing world class infrastructure with best medical facilities to needy children. I was deeply impressed by the enthusiasm at this temple of healing, which motivates me to work harder and continue to be a part of the mission that Sai has started. I really feel blessed and am heartily thankful to the divine for selecting me for the mission, to showers his blessings to my fellow citizens and the little hearts through the tenet - “LOVE ALL, SERVE ALL”.   DR SATYAJEET KUMAR MD (Pediatrics), FNB (Pediatric Cardiology) Junior Consultant, Dept of Pediatric Cardiology Sri Sathya Sai Sanjeevani International Centre for Child Heart Care & Research, Palwal, Haryana Hailing from Bihar, Dr. Satyajeet pursued his MD Pediatrics in his home state at Patna Medical College. He then worked as a consultant neonatologist, during which time he came to know that many babies go undiagnosed and die of CHD without treatment. This prompted him to pursue FNB Pediatric Cardiology. He completed his PDCC from Frontier Lifeline hospital and served in the prestigious KM Cherian Heart Foundation, Chennai till 2013. Before joining Sri Sathya Sai Sanjeevani, he worked as a visiting pediatric cardiologist to many local centres in his native place. He felt a calling to serve needy and underprivileged pediatric cardiac patients, and gave up his well-established clinic in Patna to join Sai Sanjeevani.   Dr. Satyajeet enjoys the atmosphere and environment of selfless service in Sai Sanjeevani, and says that the satisfaction he gets when he meets patients and attenders post-intervention or post-operation is priceless and cannot be quantified. This has more value to him than a lucrative career. He wishes to spread awareness amongst his circles and the extended pediatric cardiac community about Sai Sanjeevani, particularly on how it succeeds in delivering high quality and authentic service completely free of cost.   AN ARMY FATHER'S FIGHTING SPIRIT Gabbar Singh is part of the 72nd Batallion of Central Reserved Police Forces and is now posted in Rajouri Range in Jammu and Kashmir as a Constable. Originally from Agra, he lost his wife at time of their second child’s delivery. As fate would have it, it turned out that his first child Krishna was detected with a Congenital Heart Disease. Struggling to cope with the myriad of challenges, he even considered giving up his life. That’s when one of his colleagues mentioned about our hospital. He took leave and after 3-4 months of persistent follow up, Krishna had his surgery performed in Sai Sanjeevani, Palwal. He had a VSD closure and SAM excision through an operation that took almost 8 hours and is now recuperating in the Post-Operative Ward.  This is what his father had to say “I had almost lost all hope in life and then God showered his grace on my little child. I had almost spent 3 lakh rupees in private hospitals but I was not at all convinced about the treatment. That’s when I resolved that if it all my child has to get back his life, it has to be at Sai Sanjeevani. I am so grateful to everyone here. I have found hope in my life. Thank you” A RARE MEDICAL CASE Total anomalous pulmonary venous return (TAPVR) and  Aortopulmonary window (APW)           Little Gudiya presented at Sri Sathya Sai Sanjeevani, Naya Raipur with complaints of breathing difficulty and poor feeding.  She weighed only 3Kg at 4 months of age. Echocardiogram revealed that she had a large hole between the two great arteries arising from the heart. This defect is known as Aorto-pulmonary window. In addition, pure red blood from her lungs was coursing abnormally and mixing with the impure blue blood. This abnormal pathway was obstructed, further complicating her condition. In medical terms, this condition is called obstructed total anomalous pulmonary venous connection (TAPVC). The combination of these two complex lesions namely, Total anomalous pulmonary venous return (TAPVR) and Aortopulmonary window (APW) is not previously reported in medical literature.  This is a unique situation where the lungs are exposed to pulmonary venous hypertension due to obstructed TAPVC and pulmonary arterial hypertension due to unrestrictive APW. Aorto-pulmonary window is a rare lesion representing approximately 0.2% of all congenital cardiac lesions and an active centre can expect about one case per year. Total anomalous pulmonary venous return is another rare defect with an incidence of 1.5% of all congenital cardiac lesions. Gudia underwent successful surgery on the 2nd of this month. As far as we know, this the first such operation performed ever. The team was amazed at her smooth recovery and she was discharged from the hospital after one week.  DIVINE  TIMING FROM RESUSCITATION TO LIFE Divyanshi Verma, 5 years 9 months old from UP, underwent emergency Intubation and cardiopulmonary resuscitation  at  the Cardiothoracic ICU for her cardiac arrest then she was transferred into CCU. She had a diagnosis of Severe valvular Pulmonary Stenosis & ASD. She was taken up for Balloon Pulmonary Valvulotomy. Due to technical difficulties with usual femoral venous access, Internal Jugular Venous access was used and her severely stenosed  Pulmonary valve was successfully balloon dilated. She had a miraculous recovery over the next 2 days and was successfully discharged home on the 5th day of procedure. It was only because of timely resuscitation that the child could make it to Cathlab procedure! Also, it was by His Grace that the difficult neck-line approach was possible and the procedure became easier! It was truly HIS Intervention in Divyanshi's survival!  COMMENCEMENT OF ARRHYTHMIA MANAGEMENT  SERVICES At Sai Sanjeevani, Naya Raipur Electrophysiology studies test the electrical activity of a heart to find where an arrhythmia (abnormal heartbeat) is coming from. Catheter ablation is a procedure in which a small catheter is placed inside the heart (via a leg vein). The catheter has a 4-8 mm metal tip through which radio-frequency (or cryothermal) energy is delivered to selected parts of the heart. The area to ablate is selected primarily by two simple strategies: vector analysis of how the arrhythmia activates the heart (ie north-south, east-west) and secondly, by moving the ablation catheter in that direction. The 4-8 mm ablation lesions can eliminate rogue cells causing palpitations. Electrophysiology study and Radio frequency ablation was performed for the first time in the Cardiac cath lab at Sri Sathya Sai Sanjeevani hospital, Naya Raipur on 21st and 22nd April 2018. All 3 patients had WPW syndrome with left sided accessory pathway (an additional connection) for conduction of cardiac  impulses. The procedure was performed by Dr Atul Prabhu, Dr Deepak Padmanabhan, an adult electrophysiologist at Jayadeva Institute of cardiology, Bengaluru  ably assisted by cath lab staff and Mr Monotosh Mukherjee, technologist from Abott India who provided equipment assistance free of cost. The patients were successfully discharged the next day and have been off their regular medicines and free of their symptoms since. It is definitely a great feeling of relief for such patients. Hitherto this procedure was done in state of Chhatisgarh by experts visiting from other cities.  Henceforth it will be done in the divine premises of Sri Sathya Sai Sanjeevani hospital completely free of cost. ANDHRA PRADESH LAUNCH OF DIVINE MOTHER & CHILD PROGRAM Launch of Divine Mother and Child Health Programme in West Godavari. Dr Sridevi, an Obstetrician originally hailing from West Godavari and practising now in Hyderabad led a team of volunteers to start the screening programme for pregnant women in Chebrolu Primary Health Centre - PHC in West Godavari District. A total of 20 women were comprehensively screened and Nutritional Support in the form of Sai Protein was given to all the mothers. It’s heartwarming to know that a team consisting of Dr Sridevi and Volunteers travelled all the way from Hyderabad to flag off the programme. KARNATAKA SCREENING OF PREGNANT  WOMEN  April 2018     A total of 1432 mothers were screened this month in Chikballapur and Tumkur District. All the critical and High Risk cases of DMCHP in Chikballapur district will now be under her review. This will greatly help in delivering quality care for the mothers at their doorstep.  HARYANA CHHATTISGARH BLOOD  DONATION  CAMPS On  the  occasion  of  “World  Health  Day”  –  7th  April  2018, a Blood  Donation  camp  was organised at Sri Sathya Sai Sanjeevani Hospital, Naya Raipur. The  World  Health  Day  is  a  global  health  awareness  day  celebrated  every  year  on  7th  April  to  draw worldwide attention to the subject of major importance to global health each year, with a new theme every  year.  This  year  the  theme  focussed  on  “Universal  Health  Coverage  –  Everyone,  everywhere  – Health  for  All”, which resonates completely  with the essence and spirit that Sai Sanjeevani  hospital is based  upon, “LOVE  ALL,  SERVE  ALL”.  Taking this opportunity to create  awareness  among patients’ families  and hospital staff about the importance   of   blood donation and eliminating misconceptions  around  it,  Department  of  Public  Health  &  Research,  organised  a  blood  donation camp in the hospital premises on 7th  April 2018. City Blood Bank, Raipur assisted in conducting the camp.  Dr.  Manoj  Lanjewar,  Head,  City  Blood  Bank  was  present  throughout,  brought  necessary equipment, blood bags and was assisted by his 8 team members including blood bank technicians. The camp was inaugurated amidst the chanting of Veda and Sai Gayatri mantra by Dr. K L Gupta, Dr. Lanjewar,  Dr.  Atul  Prabhu  and  Mrs.  Manokamana,  Principal  SSSSINAHS.  The  vivid  presence and active involvement of hospital staff, nursing students from Sri Sathya Sai Nursing college, volunteers from Sai Samiti, Badhte Kadam and patient attendants infused the environment with enthusiasm. Nursing students, staff and volunteers helped in the overall co-ordination and guiding donors through the registration process. Certificate  of  appreciation  was  handed  over  to  all  successful  donors  along  with  refreshments.  An innovative Selfie corner with placards was set-up for appreciating all donors and taking photographs. There were total 87 registrations, including 3 rejections.   Of 84 successful donors, major participation was from patient's family members – there were 36 donors who were related to children admitted for surgery/cath intervention or already operated during that week   22 donors were from OPD patient family members   7 Army jawans – interacting with this group was a humbling & inspiring experience for the team   10 Hospital staff   9 Female donors   9 Volunteers The blood donation camp concluded at 5:00 pm with a vote of thanks and Mangala Arati.   DAILY PARENTAL OUTREACH SESSIONS SAI SANJEEVANI , NAYA RAIPUR Weekly health education sessions had been held for patient attendants/families in Sai Sanjeevani Hospital since November 2017 to increase awareness about nutrition, hygiene, sanitation, safe water, etc.  After discharge from hospital, the child who has received the "Gift of Life" and his/her parents go back to their community and for many, it is a place lacking essential health commodities and services. The impact of Sanjeevani can truly be spread wide and far when we reach out to a child who has benefited from here and then the parents practice and spread safe sanitation practices in their village and thus these sessions are now held on a daily basis looking at the vast need and potential impact that can be made through these sessions. These sessions are conducted by a core team of 40 staff members and volunteers at the hospital. There is a huge potential for the parental outreach program to spread it wings and soar high. As a country, there are a lot of economic and social gaps which affect the education and awareness levels across the nation. As we further dwell into the rural parts of our country, we observe that basic awareness and knowledge of the most crucial things like drinking water and sanitation aren't available. There are organisations working on these aspects across the nation, but as we are aware of the volume of the problem and the available resources, the gap remains. The parents who come here to Sanjeevani are our team at the village level. If these parents are trained and made to understand the significance of issues like water and sanitation, menstrual hygiene, proper disposal of waste, hand and oral hygiene, infection control and wound care, importance of education, age of marriage, planned pregnancy, spacing between pregnancies and family planning, then we would have started "impacting" the country at large.  CHILD SCREENING AT SAI SANJEEVANI NAVI MUMBAI On the occassion of World Heart Day, we conducted screening and counselling for kindergarten school children and their parents at the upcoming Sri Sathya Sai Sanjeevani Centre for Child Heart Care in Navi Mumbai. 40 children were screened for heart murmurs and health advice given to parents. Sai Sanjeevani, Navi Mumbai team screened 100 students in Navi Mumbai at a Government School. One Child was detected with a Heart Murmur and has been registered for surgery at our hospital.  Sanjeevani Sahayaks    April saw the launch of Sri Sathya Sai Sanjeevani, Palwal’s “Sanjeevani Sahayak” (Sanjeevani’s Helpers) programme. It is said that ‘one good deed begets another’. Many patients feels immensely grateful to the hospital for the ‘gift of life’ their children have received totally free of cost. Several wish to give back to the hospital in some way or another. One such case is that of little Krish’s parents, Sumitra & Mukesh Morwal, the first Sanjeevani Sahayak of Sanjeevani Palwal. While their tiny tot was being operated upon, the parents took it upon themselves to offer their help wherever they could. Krish’s mother served by making roti’s in the Sanjeevani Canteen ‘Naivedhyam’.   Once the child was discharged, they even took the time to go around the pre-operative ward to counsel other parents who were awaiting surgery for their children and give them some much needed encouragement. Who better than a parent whose child was in a similar situation not too long ago than to deeply empathise with and counsel other parents? With Divine grace, their seva to His hospital has resulted in a beautiful movement amongst patient attenders. Dozens more have followed in the footstops of Sumitra & Mukesh. These Sahayaks have served in the canteen, front desk, and even helped to pack food supplies for the schools that benefit from the Annapoorna Breakfast seva programme.   A new beginning Aradhana Diwas at the upcoming Sai Sanjeevani, Navi Mumbai   Aradhana Day - 24 April 2018  was observed at Sai Sanjeevani, Navi Mumbai. All the workers and staff at the construction site participated in the bhajans and were shown a video on Bhagawan's Health Mission.This event brought a sense of camaraderie and purpose to the hard work they are putting in to ensure that the Hospital will start serving the children of this country this November. The Local Sarpanch also participated in serving dinner Prasadam to one and all.  Aradhana Diwas at Sai Sanjeevani , Naya Raipur   Staff and students of the Sai Sanjeevani Nursing College invited families of little children to celebrate and rejoice the FORMLESS DIVINE .
