A FIGHT AGAINST ALL ODDS
A Story from the South Pacific as shared by Dr Maryanne Kora’ai, Principal Medical Officer, Junior Pediatric Consultant, Colonial War Memorial Hospital Fiji

Baby Peter, as he is called is a cheerful 2 years old Solomon Islander.

Peter has complex CHD, diagnosed by a foetal echocardiogram done while his mother was studying in Australia. Peter's parents were given the option to terminate the pregnancy or deliver baby in Australia where an urgent lifesaving surgery is an option.

It was a heart-breaking news for the young parents. Having surgery in Australia would cost them a fortune, hence they decided to return to Solomon Islands where Peter was born.

Peter was expected to live for few months without surgery. His parents were determined not to give up on their son just because of the costs associated with surgical treatment abroad.

Their struggle to fundraise was boosted when Julie, a friend from Australia, joined them in the fight and created a GOFUND ME page. With support from other local friends they also did a lot fundraising locally. Many kind souls generously contributed, hence, at 9 months of age, Peter went for a BT shunt surgery at the Fortis Escort Heart Institute in New Delhi.

An echo was needed at 6 months post-surgery. Through “Keep Baby Peter’s Heart Pumping” campaign, he was able to go for review with a cardiologist in Australia. He advised that Peter needs a "Glenn Shunt", which would cost them ~ $AUD120, 000 if done in Australia.

By then, Julie, who was working in Solomon Islands moved to work in Fiji. She heard and enquired about Sanjeevani team's visits to Fiji. Things seemed to work in Peter’s favour, soon Lyn and Peter joined Julie in Fiji as they wait with great hope for the review with Sanjeevani team.

On the day of surgery, as Peter was taken into OT, Lyn looked on with mixed feelings and tears in her eyes. Given the complexity of her son’s condition, anything could happen. However, she was comforted knowing that Sanjeevani team has so much cardiac surgical experience. This was proven when the doctor emerged with a big smile and told her the operation was successful.

Lyn is so happy and is thankful to each and every one who had helped to give Peter the ‘Gift of life’. She sees each help as God’s act of love and kindness and she would always be grateful for that. “You have all given my little warrior another chance to see the rainbow that he loves so much.”
Sanjeevani Cures Twin Troubles

Praveen told us, "two girls and two boys are just the same. These are my two everything" when he came to the doctor's chamber for consenting cardiac surgeries of his twin daughters, Swarali and Sharvari.

A daily wage labourer, he was thrilled to know he had twin girls. Six months after their birth  after a routine check-up, he got to know that the younger kid, Sharvari had CHD. The process of tests and ECHOs led them to Sanjeevani and a date for surgery in the year 2019. She was medically managed till 2017, and that's when a more complicated issue was suddenly diagnosed in Swarali, which was almost inoperable.

The parents were shattered. This was a blow beyond anything according to them. They left the outcomes to their fate when informed about the risk of the surgery of Swarali and decided to go for the same.  An undetected riskier CHD in the older twin and a manageable condition detected earlier for the younger twin, the family was ready to take it all.

Swarali was still recovering in the ICU when Sharvari was operated upon. Swarali was discharged after about 3 months and Sharvari had already come for her post-operative visit during her sister’s stay in the ward.
Today both the daughters are healthy and have gone home for recuperation and the family is complete with smiles.
Saved in the Nick of Time

Four year old Piyush Kharaliya from Palwal - a blue baby since birth - visited our OPD at 3 months of age and was diagnosed to have mixed type of TAPVC (Total anomalous pulmonary venous connection). Piyush also had an ASD, which was  enough to help sustain life then and was thus given an elective date for surgery.

A few days after returning home, Piyush suddenly developed signs of distress and wasn’t feeding well. Hence, his parents decided to bring him to the hospital well ahead of their planned revisit date. Piyush was brought to our hospital on 6th May afternoon and as soon as registration was complete, he was found gasping and was arrested in front of the registration counter.
The cardiology team was quick in initiating resuscitation and brought back life in him.
The child was shifted to the ICU where he was reassessed and found to have developed an obstruction at the level of ASD, which warranted immediate surgery. He was stabilised on ventilator and successfully underwent surgery on 7th May and was discharged  on 15th May. Piyush was directed by none other than the Divine Himself to reach the hospital, as he was destined to live today.
Mixed type of TAPVC contributes to less than 10% of TAPVCs that come across in practice. The pulmonary venous channels get separately connected to at least 2 different systemic veins in a mixed type. In this particular case of mixed TAPVC, the right sided pulmonary veins along with the left lower pulmonary veins drained to the SVC through a short right vertical vein and the left upper pulmonary vein drained into the left innominate vein through a left vertical vein. The ASD was the site of obstruction which created an emergency and brought him to us.

Piyush underwent a Wardens repair with rerouting of the right sided and left lower pulmonary veins to Left atrium and a rerouting of left upper pulmonary vein to Left atrial appendage. He had a smooth post-operative period was discharged 8 days later. Truly speaking, Piyush was saved in the nick of time!
14 Years of Hope and Wait
Fourteen years of wait did not deter parents of Dinaomi Tinai. A child diagnosed with TOF when she was 6 months, it was a journey of 9 hours by land and sea to reach the capital of Fiji for even an ECHO study. Considering the fact that Fiji doesn’t have pediatric cardiac services and the fact that this was a family of farmers where two square meals were more important, it was a long wait of 14 years for the family to see the hope of surgery and Life for the child. Her doctor visits were also irregular owing to their affordability. There were many cardiac teams visiting the islands but Dinaomi was refused each time because of the risk involved. The family took last leap of faith when they undertook the arduous journey of 9 hours again in April 2019 to Colonial War Memorial Hospital, Suva Fiji for the Mission undertaken by Sri Sathya Sai Sanjeevani Hospital. Dinaomi was operated upon successfully and the family couldn’t thank the team enough. Dinaomi’s mother is overjoyed and she said that words are not enough to express her gratitude. “I thank you with all my heart and may God bless you all.”

Expanding the Lists of Firsts

Eight year old Ishu Devi, from Kaithal, Haryana, experienced frequent chest infections and required frequent hospitalizations during her early childhood. She underwent detailed cardiac evaluation at Sanjeevani, and was diagnosed with a very peculiar type of VSD. Her main VSD shunt was in Inlet muscular level with LV to RV side entry—exit points at same level; but the RV had additional exit mouths at mid-muscular and Apical muscular levels too! This is an extremely rare type of VSD, labelled as “Intrafascicular VSD”.
She was admitted for device closure of the said defects after detailed counseling of parents. Upon LV ventriculogram, Intrafascicular VSD was delineated well: it measured 7 mm on LV side and had 3 mouths on RV side, each measured 6 mm(inlet muscular level),  7mm (mid muscular level) & 5mm(apical muscular) levels. Lifetech Duct Occluder (8/10) was deployed across the main defect; post device LV ventriculogram revealed complete occlusion of all remaining Intrafascicular shunts too, in addition to the main defect!
This was a very tricky type of successful percutaneous device closure of a complicated Intrafascicular VSD, a VSD which can be quite difficult to close surgically, in view of its complex nature. This has been a first of its kind of Device closure of VSD, which is a rare feat achieved in Pediatric Cardiac Interventions till date! Sai Sanjeevani continues expanding its Lists of Firsts!

INTRODUCING MEDICAL TEAM MEMBERS

Dr Arya James
MBBS, DA, DNB

Junior Consultant, Pediatric Cardiac Anaesthesia & Critical Care, Palwal

Although a native of Kerala, Dr Arya pursued her education in various cities, including Kolkata, Delhi, Bangalore and Mysore. She received her MBBS from St. John’s Medical College in Bangalore and then a Diploma in Anaesthesiology from Mysore Medical College & Research Institute. She then completed a DNB in Anaesthesiology at Sri Sathya Sai Institute of Higher Medical Sciences, Whitefield, Bangalore.

Dr Arya chose to delve in the world of anaesthesia as she believes anaesthesia is an elective to rescue people who are dying, and a pain relief for people in severe distress. According to her, it is a field that uses both brains and hands. Having worked in other hospitals, she believes Sanjeevani has the most balanced yet relaxed approach to critical care.
She shares, “It’s good to be working at an institute where everyone is clear about what our common goal is. Everyone works in their different ways, and at their different jobs, but all towards the same goal: optimize and provide the highest level of care to an underserved population. This makes me very happy to see.
Dr Siddharth Amboli
MBBS, DNB - CTVS

Junior Consultant, Pediatric Cardiac Surgery, Palwal

Hailing from Maharashtra, Dr Siddharth completed his MBBS degree from his home state at MIMER, Pune. Passionate about the human heart & its physiology, Dr Siddharth was certain that he would take to the path of cardiac care. However, he was confused between cardiology and surgery. After a brief stint in plastic surgery, Dr Siddharth knew that his calling lay in cardiac surgery.
He gained admission in the 6-year DNB CTVS course at Sri Sathya Sai Institute of Higher Medical Sciences, Whitefield, Bangalore. While he was there, he found pediatric cardiac surgery challenging, until he met his inspiration, Dr Krishna Manohar (currently Senior Consultant, Pediatric Cardiac Surgery, Palwal). He suddenly felt that pediatric cardiac surgery became simplified and this inspired him to join Sanjeevani after finishing the course.
He shares, “This institute has people who do not have any personal agendas. Everybody has the same motive to work, treating maximum number of sick hearts. Only when the entire team moves with the same purpose towards their goal will they be able to achieve it. If people have given up their comforts and come to a place like Palwal, you can be well assured that they are here only to serve. Working & learning in such an environment has been a pleasure.”

Life at Sai Sanjeevani

As we always fondly say in Sanjeevani, one might be going from Sanjeevani, but one can never not have Sanjeevani as a part of them. Once a Sanjeevani member, always a Sanjeevani member. We had to bid adieu to two of our dearest Sanjeevani members, Dr Sanjukta Bose, Fellow in Pediatric Cardiology and Dr Nishith Bhargava, Consultant in Pediatric Cardiac Surgery. It was not easy for any member of the Sanjeevani Family to say this goodbye, but as life takes us to places in our journeys, Sanjeevani Members take with them the values of Sanjeevani and will always remain a part of us. We wish them all the best in their future endeavors and lots of love from the entire Sanjeevani Family.
Dr Petronilla Tabansi from Nigeria has completed her Fellowship in Pediatric Cardiology in Sanjeevani, Atal Nagar. It was a very touching farewell as Dr Pets, as she is fondly called, spoke to Sanjeevani Family and shared her experiences in the past year. She mentioned that a part of her will always carry Sanjeevani closest to her heart. Bidding her Goodbye was not easy and we at Sanjeevani wish her well and hope that her Fellowship will contribute to the children of Nigeria in helping them screen and detect Congenital Heart Disease. We wish her all the best in her future endeavors.
